Bone Marrow Manifestation of Hodgkin's Disease

Abstract
In Hodgkin`s disease, positive bone marrow biopsy indicate the disseminated stage and so obviates the necessity of a staging laparotomy. Histological diagnosis of Hodgkin`s disease in the bone marrow may be difficult due to the heterogeneity of bone marrow finding and the diligence required to find diagnostic Reed-Sternberg cells. In this paper, we describe systematically the pathologic changes of positive marrow and reactive changes and review patients` clinical data to determine if alteration of hematological and clinical parameters may be predictive of bone marrow involvement. From June 1989 to September 1991, 26 bilateral bone marrow biopsies were performed on 12 patients with Hodgkin`s disease. Four patients exhibited bone marrow involvement. These were one type each of nodular sclerosis, mixed cellularity, lymphocyte depletion type, and one was unclassified. The bone marrow findings of the nodular sclerosis type and mixed cellularity type are focal cellular and those of lymphocyte depletion type and unclassified type are diffuse cellular. Elevation of serum alkaline phosphatase, depression of blood cell count and manifestation of B symptoms were revealed in all patients with positive marrow and clinical stages of three patients with positive marrow were lIIA or turther. Only four patients of eight with negative marrow are revealed at least one of above parameters. The data indicate that these parameters are useful in predicting bone marrow involvements in Hodgkin`s disease.
